論文名稱Developing a Competence Scale of Advance Care Planning of People with Dementia for Healthcare Providers Working in Long-Term Care Facilities

備註Objectives：The purpose of this study was to develop an instrument which can be used to understand the knowledge, attitudes and skills of healthcare providers in ACP for PWD in long-term care facilities.
Methods：We adopted Iceberg Theory as the theoretical foundation to design the scale. A 30-item scale was developed to assess the knowledge, attitude and practice of healthcare providers in ACP for PWD. Developing this scale through three stages included item development, content validity and psychometric testing. Subjects were recruited for 18 long-term care facilities. Internal consistency of the items and exploratory factor analysis were used for construct validity.
Results：164 healthcare providers were enrolled. The CVI of experts was 0.99-1.00. Cronbach’s alpha coefficient was 0.882 of pilot study. The internal consistency reliability of the scale was 0.893, and the subscale of knowledge, attitude, and skill were 0.782, 0.871 and 0.921, respectively. Through EFA, the item accounted for 47.1%, 58.5%, and 58.5% of the total variance for knowledge, attitude, and practice, separately.
Discussion/Implications：This scale was supported by theory with good reliability and validity. It can be used to for administrators or educators assessing competence of healthcare providers to assist PWD with making an ACP in long-term care facility. In addition, a suitable education program can be planned. Hopefully, healthcare providers can discuss ACP with PWD early and complete ACP so as to reduce ineffective medical and social costs.
